Sub: Notification of Fresh empanelment of Private Hospitals & Diagnostic Centers under continuous empanelment scheme of CGHS, Meerut
In reference to Directorate email dated 31-12-14 & Notification issued by this office lette No. 9-4/15-CGHS/MRT/ESTT dated 07-01- 2015 & corrigendum dated 19-02-2015 and last date o application submission on 28-02-2015 for continuous fresh empanelment of Private Hospitals 6 Diagnostic Centers under CGHS Meerut, a committee was formed with two CMOs headed b’ undersigned including Office Superintendent. Inspection for authentication of documents submittei was carried out by Transparency officer, CGHS Meerut, Office Superintendent CGHS Meerut & Vigilanci Officer CGHS Meerut. The qualified list of Hospital is according to category and purpose attached a given below (on the recommendation of Hospital Committee) for One Year or Fresh Empanelmen whichever is earlier.
1.The Hospital who have qualified to be empanelment under CGHS, Meerut have submitted the Draf MOA with CGHS along with performance Bank Guarantee are approved for empanelment of Privati Hospitals under CGHS, Meerut with effect from date of notification of the OM.
2.The empanelment shall be for a period of one year from the date of notification or till nev empanelment process whichever is earlier.
