Subject: – Revoking the suspension of empanelment of Stars Dental Clinic, A-14, D.K. Road, Mohan Garden, Uttam Nagar, New Delhi 110059.
Sir,
With reference to the above mentioned matter. the Stars Dental Clinic, A-14, D.K. Road, Mohan Garden, Uttam Nagar, New Delhi 110059 was suspended from empanelment under CGHS Delhi/NCR vide Office Order No. 6-82/के.स.स्वा.यो. (मुख्यालय)/शिकायत प्रकोष्ठ/2021/153-84 dated 28.01.2022 for a period of six months or till further orders. After careful examination of explanation given by the concerned Dental Centre, it has now been decided to revoke suspension of empanelment of Stars Dental Clinic, A-14, D.K. Road, Mohan Garden, Uttam Nagar, New Delhi 110059 under CGHS and the HCO is permitted its empanelment w.e.f. date of issue of this Office Memorandum.
2. The other terms and conditions of empanelment shall remain unchanged.
